Objectives:To investigate the use of sildenafil citrate and sildenafil citrate plus diltiazem in patients with erectile dysfunction (ED) who were evaluated for treatment-resistant ED by the FDA-approved method of venlafaxine, which has a lower incidence of adverse events than sildenafil citrate.
Design:This was a single-blind, randomised controlled trial.
Setting:A total of 24,086 patients (aged 18 to 64 years) with ED who were being treated with sildenafil citrate, diltiazem, or combination therapy were randomised to one of four treatment groups, either sildenafil citrate or diltiazem, or placebo. The patients were randomly allocated to each of the study groups to either sildenafil citrate, diltiazem, or placebo at the same time as the other treatments.
Intervention:Patients were recruited from patients who had been treated with sildenafil citrate or diltiazem and had been assessed for treatment-resistant ED by the FDA approved method of venlafaxine. Patients were also assessed for treatment-resistant ED by the FDA approved method of venlafaxine.
Outcome measure:The primary outcome was ED at 24 months, which was defined as the presence of at least one ED symptom, or ED at baseline (including a positive urine drug test, a positive urine flow test, and a positive serum erection test). The secondary outcome was the proportion of patients with ED that responded to treatment.
Patients were randomly allocated to sildenafil citrate, diltiazem, or combination therapy at baseline to either sildenafil citrate, diltiazem, or placebo. Sildenafil citrate or diltiazem was given once daily, and patients were assessed 24 months later.
Main outcome measure:The primary outcome was the proportion of patients who received treatment-resistant ED (defined as the presence of at least one ED symptom or ED at baseline, or ED at the end of the 24-month study). The secondary outcome was the proportion of patients with ED that were treated with treatment-resistant ED by the FDA-approved venlafaxine (n=4,802).
Results:Of patients randomised to sildenafil citrate, diltiazem, or combination therapy, 11.7% were randomised to treatment-resistant ED, and 6.3% were randomised to treatment-treated ED. The proportions of patients with ED that were treated with sildenafil citrate, diltiazem, or combination therapy were 27.1% (n=3,582), 14.8% (n=2,723), and 7.1% (n=1,162), respectively.
Conclusion:The use of sildenafil citrate or diltiazem for treatment-resistant ED by the FDA-approved method of venlafaxine in patients with ED has been found to be superior to the use of sildenafil citrate or diltiazem alone. The use of sildenafil citrate or diltiazem for treatment-resistant ED may be more common than sildenafil citrate alone and diltiazem alone. The most common adverse events that occurred in patients who received treatment-resistant ED were headache, diarrhea, flushing, dyspepsia, and insomnia.
